I’m really on the fence about this game. I hear a lot of talk about the setting and the style but not much about engaging or addicting gameplay
This game reminds of a classic Russian novel I tried to read once, five hundred pages of nothing happening while everyone is miserable. Then someone dies and it’s over.

The way you say "Time for Video Games…" at the start lives rent free in my head any time i start any game. It took me forever to find it again!
Worth noting that the game is very subtle in how it gives you quest solutions. For instance, mandalore complained about the 50 vitamins quest but up until that point you had been given over half of that. However, the Bachelor playthrough teaches you that the yellow "alpha" blockers are basically useless so you sell them in your haruspex playthrough only to get blindsided by needing 50 of them for a quest. Also you don't need to give the kid 30 clips of ammo if you don't kill the kid on the first day, or you kill the kid that asked for the ammo, or if you talk to the bachelor, he tells you he has the ammo in his room that you can go get
As an Eastern European I found a single thing I disagree with in your review. We are all sad here.

I always come back to this video, one of my absolute favorites of yours. I would never have the patience to play this game but I found the story so fascinating and explained very well/interestingly.

The Void, another game from the same developer is one of the most beautiful, atmospheric and haunting games I've ever played, while at the same time being one of the most clunky games I've ever played.
I always loved the Pathologic games because it's genuinely relatable. I open up a game and I'm like "dude this is probably what I'd be doing if things were just a LITTLE worse"
I love the story, I love the gameplay. It feels like a weird reflection of my own existence.

Thankfully there is a good explanation for why endings like this are infuriating.
Because an illusion only works, while you are convinced. Once the illusion it shatters, what you are sitting with is pieces, and not what you had before. A lot of meaning stems from the meaning we put into something, so an "it was all a dream" ending, amounts to the creator(s) telling you "what you just put your feelings into didn't matter", and few things hurt like being told what you care about is meaningless.
Even when the point is to shatter the illusion, you have still shattered the illusion. It is like a game that is purposefully bad, or making a machine that causes oil spills. Sure, it works exactly like intended, but there is a good reason why those things are, for the most part, unintended mistakes, and not the point.

The idea of this game is simpler than its title makes it out. The idea of pathology being involved makes you understand not only the suffering of sickness but the suffering of the people in the town via, Pathos, which literally is revealed at the end, the author wants to make you feel something, specifically the emotions that the author wants you to feel. The author knew what you were going through and how it made you feel from the start, it was your predestined path. Like an actor on a stage in a play, you play your part and that is what you do, you don't do anything else. It's a story and you are a character in it, but not only are you a character in it you are a character out of it. You get the one thing that no one else really gets in the tale, you eventually get to see it from the perspective it was meant to be seen, while all the others in this story never really do. The unreliable secondhand half-truths are fed to you by the people in the story however, it is not necessarily a lie, it's what they understand as truth with moral relativism. It is only in the end that you can see what the story is about. So when playing this game you are playing it as both the viewer of a film and also a character in the story, you don't have the knowledge of the entire thing before you finish the book.
We have a joke. Russian literature is full off torments. Ether charecter, or author, or reader is tormented. If there are tormented all of them – it is a masterpiecce of Russian literature. And this game is the same.
no matter how much russian criticize throwing lives away, they still do it for their tzarfuhrer with no questions. they're lost.
Do people think this is clever? Really? The authors come out to ask why you bother to get invested in a story demonstrates a complete lack of what I would think is basic human story telling instinct.
