Eleonoras twinblade +10. 50 dexterity and 50 arcane will do the job. Lord of bloods exultation, rotten winged sword Insignia or Millecents prosthesis (if its your first playthrough, if not, preferably use both), shard of Alexander + whatever fits best for your build (if you didn't use the 2 talismans from Millecent). Add the white mask to your armor too.
For the physick I had green burst and dex tear. Use the twinblades ability properly and you should kill her in less than 5 minutes. Golden vow and Flame, grant me strength would be helpful but are not necessary.
Personally I had 40 vig (way to low, first part of waterfowl killed me everytime if i took it straight to the face), 50 end, 55 dex and 45 arc with 12 health pots and 2 fp pots (+12). Close to 160 levels i think. Try and have big poise too ( I had 87), it helps a lot. I also used Radahns great rune but with this method I don't think that's necessary.
It will be a good thing to know how to dodge waterfowl and clone attack. The chances of you not having to deal with waterfowl for both fazes are kind of low.
Try to bait her by staying to mid range and by going back and forth. As soon as you see her jump, immediately start running backwards. Waterfowl consists of 4 attacks. (While locked on) You will probably dodge the first part just by running backwards. After that, go towards her, dodge forward and then again go forward and dodge forward. For the last part immediately dodge backwards and that's it, with good timing you should get hit maybe once if not at all. If you are close to her, brother just pray and spam the dodge button till the first part of the attack is done. Then you can follow the previous steps.
The clone attack consists of 7 parts. For the first 2nd you have to dodge backwards. For the 3rd attack dodge forwards and keep running forwards. This way the 4th attack will not hit you (no dodge required). As soon as the 4th attack goes above you, dodge forwards for 5th and then again forwards for the 6th attack. Lastly, when malenia moves her sword downwards, dodge forwards and that should be it. Keep in mind that you should be at mid to longe range for this to work properly.
Now back to fighting her. With the small stuns plus the blood loss and maybe even a stagger, you will be in faze 2 in no time. If you spam the skill move carefully and fast enough, there is a good chance that you will proc her second bloom. For her bloom stage, it's worth to take <=half rot and do a full skill move combo. It's really hard to stagger her in faze 2 but the bleed from the skill move is powerful enough. However, it's still a really hard boss fight because you still have to dodge and get lucky with her attacks. It's WAY EASIER with this method, yes, but you still need to be careful.
Other methods are bloodhounds fang+10, rivers+10, double seppuku on a nagakiba +25 and an uchi +25. These are the ways I tried to deal with her (around 200 deaths till I used the twinblade properly). I also read from someone that reduvia +10 is good too but she dodges the skill move 90% of the time. Maybe I didn't use it properly ¯_(ツ)_/¯.

Melania is playing so shitty against him in the 2nd pahse, for me, i cant even get a hit on her, cause she is constantly in the air, immidiately after she lands she starts with waterflow dance, then she just keeps spamming air jump into slice and then air again with the phantoms. Its ez to win against her if she is modded not to do those constantly
