During Apollo 13 the Astronauts returned to Earth from Lunar orbit while inside the LM which provided very little protection from the deadly radiation of the Van Allen belts; furthermore, the LM and Command Module took a fairly direct path through the Van Allen belts during Apollo 13 on the return trip to Earth.
